1、              境外人员健康体检验证：

1) 境外人员须提交所在国公立医院签发的体检表，如系私立医院签发的体检表须经所在国公证机关公证，其中血检（须包括艾滋病、梅毒检验）和胸部X线检查须提交原始检验报告（英文或中文），体检表自签发日起六个月有效。

2) 验证时间：周一至周五的上午9:00—12:00（除节假日）。

3) 验证不符合国家有关规定，必须补检和复检。

4) 体检验证有效期6个月。

2、              境外人员健康体检：

1) 时间：每周一至周五的上午9:00—12:00（除节假日），体检者须空腹，带本人护照和4张近期免冠2寸彩色照片。

2) 取证时间：体检日后第二个工作日的上午9:00—12:00

下午14:00—17:00

1.      Verification of Physical Examination Record

1.1     Necessary Documents（must In English or Chinese）

1.1.1        Physical Examination Record issued by a public hospital (or a health center).If the Record was issued by a private hospital, the medical practitioner’s signature and special notarization are also needed.

1.1.2        Original Report of blood test for HIV and SYPHILIS.

1.1.3        Original Report of chest X-ray test for TUBERCULOSIS.

1.1.4        The validity of the above-mentioned documents is six months.

1.2     The above requirements must be followed. Otherwise, according to Chinese regulations, necessary physical exams must be performed in International Traveler Health Center designated by CIQ Chongqing.

1.3     The certificate of verification for physical examination record is issued by CIQ Chongqing after satisfactory verification, valid for six months.  

1.4     Time for verification: 9:00—12:00, Monday to Friday.

2.      Physical Examination

2.1     EMPTY STOMACH, PASSPORT and four recent standard 2-inch colour PHOTOGRAPHS are needed.

2.2     Time for physical examination: 9:00—12:00, Monday to Friday.

2.3     Issuing time: The second work day after physical examination. Office hours:9:00—12:00,14:00—17:00.
